## How it works

The checklist comes from the guide 'What to bring to an ADHD evaluation' on this site, which follows how clinical ADHD evaluations are typically structured: a clinical interview, standardized rating scales with multiple informants, and direct measures of attention.

Each item appears only when it applies: school records and teacher forms are listed for children and teens, work performance reviews for adults, and past reports only when you already have a diagnosis.

Nothing you answer is stored or sent anywhere. The three choices only decide which static checklist items are shown.

This site never publishes prices or wait times. Fee and insurance questions are on the checklist for you to ask directly.

## Questions

### What if I do not have report cards or work reviews?

That is common. Tell the clinician what you could not find; the interview and rating scales carry the evaluation. Missing records slow an evaluation, they do not stop one.

### Should I bring a copy of my old ADHD diagnosis?

Yes. A past diagnosis or evaluation report is important context even when the new evaluation is the more thorough one. The checklist includes it when you say there is a past report.

### Can I print this checklist?

Yes. Open the page in a browser and print it, or copy the generated sections into a note. The checklist changes only with your three answers, so nothing about it is personal.
